Fynl.io Forms templates
With Forms in Fynl.io you don't start from scratch: pick a template, press Use, and change any question.

In the Fynl.io portal sidebar, open Forms & surveys and start a new form. The New form page gives you three routes: Write it with AI, + Start blank and Start from a template.
Pick a template
Under Start from a template there are 38 templates (All 38). Filter them with the chips along the top: Lead capture, Contact us, Booking request, Quote request, Intake & onboarding, Feedback & NPS, Event registration, Order form, Job application, Waivers & consent, Survey and Quiz.
Each card shows the form's name, a short description, its questions and how many there are. Simple lead form, for example, has five questions.
Search
Next to the heading is a Search templates box. Type booking and only the booking templates stay, such as Book an appointment, Booking request (no calendar) and Free consultation.
Use: the full form opens
Press Use on a card. The form opens in the builder with all its questions. Across the top are four tabs, Build, Theme, Settings and Share, plus Edit / Preview and a Results button. Changes save automatically (All changes saved).
The left column lists question types such as Short answer, Long answer, Email, Phone, Number, Website / link, Address and Date. Click any question and Question settings opens so you can change its choices, logic and more.
Book an appointment
The card for this template says it takes real times from your booking calendar and books them when the form is submitted. Its questions: name, email, phone, which service, a time slot and one extra question for details.
Write it with AI
If no template fits, press Write it with AI. As the page itself says, you need to connect your own Claude or ChatGPT key under Account → AI so it can write the form from a sentence. The AI runs on your key, not ours.
Start blank
Or press + Start blank: you enter a name and email and build the rest yourself.

How many templates are there?
38, in categories such as Lead capture, Booking request, Quote request, Survey and Quiz.
How do I find a template?
Use the category chips or the Search templates box. Typing booking shows only the booking templates.
Can I change the questions after pressing Use?
Yes. The form opens in the builder, and clicking any question opens Question settings.
What does Write it with AI need?
Your own Claude or ChatGPT key, connected under Account → AI. The AI only runs on your key.
Can I start from a blank form?
Yes, with + Start blank. It asks for a name and email and you build the rest.

The AI runs on your own Claude or ChatGPT API key. You keep the key, you pay the model provider directly, and there is no markup on top of it. There is no SMS or phone calling yet, and we would rather say so than let you find out later.
